Practice Statement - Named accountable GP for all patients

From 1st of April 2015 onwards Practices are required contractually to allocate a named accountable GP to all patients, including children.

All patients at our practice have a named GP to oversee their care.

It is important that you are aware that this is purely an administrative exercise and does not change the way in which we operate or affect your ability to make an appointment or speak with any of the GPs in the practice.

What are the named GP’s responsibilities to 75s and over?

This is unchanged from 2014-2015; for patients aged 75 and over the named accountable GP is responsible for working with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient
